Heavenly Gates Hospice Inc. is a small business in Simi Valley, CA. Our goal is to provide and individualized program of physical, emotional, spriritual, and practical care for people in the last phases of a life-limiting illness, with an emphasis on control of pain and other symptoms. The program is reflective of a spirit and idea of caring that emphasizes comfort and dignity for the dying, making it possible for them to remain independent for as long as possible, in familiar surroundings. Hospice personnel respect and respond to the unique differences in family/ caregiver, lifestyle, values, and wishes.

We are looking for a Hospice Nurse to join our team and provide compassionate care to patients and their families. The successful candidate will be responsible for assessing patient needs, providing medical care, administering medications, and educating families on hospice and palliative medicine. The ideal candidate will have excellent communication skills, a passion for helping others, and a commitment to providing the highest quality of care.

Responsibilities:

• Provide emotional and spiritual support to patients and their families.
• Assess the physical, psychological, social, and spiritual needs of the patient and family.
• Provide end-of-life care and symptom management.
• Educate patients and families about hospice and palliative care services.
• Coordinate care with other healthcare providers.
• Participate in interdisciplinary team meetings to develop a plan of care for the patient.
• Document patient care activities in accordance with regulations and standards.

Pay: $150.00 - $200.00 per hour
